JOB SUMMARY: Work as part of the clinic nursing team to provide a full range of nursing services including phone triage, floor nursing, acute case management related to triage, patient education and wound care. Work is centered around the needs of patients being seen in the clinic that day. As capacity permits, the Triage Nurse may need to cover for other RN positions as well as other sites during staffing shortages.
This is an in-person position with a schedule of Monday through Friday, 8:45 AM – 5:15 PM, located on-site at our Gravenstein Community Health Center in Sebastopol, CA 95472.
RESPONSIBILITIES:
- Perform phone triage of patients calling in with symptoms.
- Provide back up to fellow Clinic Nurse for overflow and during absences.
- Complete special projects during triage down time as directed by RN Lead or Director of Clinical Operations.
- Administer medications as ordered by provider: Rocephin, insulin, valium, B12, testosterone, others as requested.
- IV starts.
- Manage/ support staff with clinic emergencies and disruptive patient behavior.
- Assist with patient transfers to ER.
- Daily triage of site Lab/DI results and manage critical abnormal lab/DI results with onsite providers.
- Assist with patient “Care Items” which could include: medication refills per protocol, pharmacy clarification of prescriptions and other items as needed.
- Provide proactive utilization management for patients with acute high-risk needs.
- Prevent inappropriate utilization of emergency room service.
- Provide Nurse Visits for designated conditions per clinical protocol (Emergency contraception, may assist with UTI in preparation for provider visits).
- Oversee Care Team Medical Assistant administration of immunizations and medications, both injection and oral within the office visit.
- Work collaboratively with Care Team to optimize communication, coordinate needed services and meet the needs of the patient.
- Work collaboratively with the Care Team to complete needed patient care items.
- Work with Care Team to actively manage the patient schedule to optimize efficiency, patient experience, and meet the needs of the patient.
- Assist in the coordination of warm hand-offs or “Transfer of Trust” between Care Team, Behavioral Health Staff, or other support services during the office visit.
- Actively seek out opportunities to assist other team members in collaboratively meeting the needs of the patient and Care Team.
- Assist in the adoption and implementation of new technologies and activities that improve patient care and clinic efficiencies.
MINIMUM REQUIREMENTS:
- Current California Registered Nurse License
- Current CPR certification
EXPECTATIONS:
- Bilingual Spanish/English strongly preferred.
- Ability to work well on a multidisciplinary team.
- Ability to work independently, problem solve and be self-motivated with acute attention to detail.
- Strong organizational skills and the ability to multitask effectively in a busy environment.
- Expected to have strong interpersonal skills and can understand and follow written/verbal instructions.
- Ability to work independently with little or no supervision.
- Excellent interpersonal skills to interact positively with patients and staff alike.
- Proficiency in using office software and electronic health record systems is beneficial.
PHYSICAL REQUIREMENTS:
- Ability to bend and move arms with sufficient range of motion to complete patient related requirements, hearing required for patient assessments, may need to assist patients onto exam table, able to lift 30 lbs., bend, kneel and twist as needed.
- Prolonged standing and occasional sitting required.
- Must be able to perform the essential duties with or without accommodations. Basic office and equipment safety knowledge and practice.
