Job Overview
We are seeking a dedicated and compassionate Physical Therapist to join our team. As a Physical Therapist, you will be responsible for assessing patient needs, developing individualized care plans, and implementing effective treatment strategies to promote recovery and enhance mobility.
Responsibilities
- Conduct thorough evaluations of patients to determine their physical abilities and limitations.
- Develop and implement personalized care plans based on patient assessments and goals.
- Collaborate with other healthcare professionals to ensure comprehensive patient care within hospital systems and home health settings.
- Educate patients and their families about treatment options, exercises, and self-care strategies.
- Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes.
- Maintain accurate documentation of patient treatments, progress notes, and discharge summaries.
Skills
- Strong knowledge of orthopedic conditions and treatment methodologies.
- Ability to develop effective care plans tailored to individual patient needs.
- Excellent communication skills for interacting with patients, families, and multidisciplinary teams.
