What You'll Do
- Serve as the primary clinical case manager for your hospice patients throughout their journey.
- Complete comprehensive assessments and develop individualized plans of care.
- Coordinate services with physicians, social workers, chaplains, hospice aides, volunteers, and therapists to ensure seamless, compassionate care.
- Manage symptoms, provide pain management education, and advocate for patient comfort and quality of life.
- Educate and support patients and families so they can confidently navigate difficult healthcare decisions.
- Build trusted relationships with hospitals, skilled nursing facilities, assisted living communities, physicians, and discharge planners to ensure smooth transitions into hospice care.
- Maintain accurate documentation and collaborate with the interdisciplinary team to deliver exceptional patient outcomes.
We're Looking For
- Active Minnesota RN license.
- Hospice, oncology, palliative care, home health, or other advanced clinical experience.
- Strong clinical judgment with exceptional communication and relationship-building skills.
- Passion for educating patients and families through complex healthcare transitions.
- Ability to work independently while collaborating with an interdisciplinary team.
- Valid driver's license and willingness to travel throughout the Twin Cities Metro.
