EPIC Health is seeking a Behavioral Health Care Manager (BHCM) to support patients through our Collaborative Care Model (CoCM). This role provides behavioral health support, care coordination, patient education, and proactive follow-up while working closely with primary care providers, psychiatric consultants, and the interdisciplinary care team.
The ideal candidate is an experienced Master’s-level Social Worker who is passionate about integrated care, patient engagement, and improving behavioral health outcomes.

Identify, enroll, and engage patients appropriate for the CoCM program.

Conduct behavioral health, psychosocial, and social needs assessments.

Administer and monitor PHQ-9, GAD-7, and other behavioral health screening tools.

Provide proactive patient outreach, follow-up, education, and brief evidence-informed interventions within professional scope.

Monitor patient progress and identify patients who are not improving or require additional intervention.

Conduct appropriate safety/crisis assessments and follow established escalation protocols.

Maintain and regularly update the CoCM patient registry.

Prepare appropriate patients for psychiatric consultant case reviews.

Communicate psychiatric recommendations to PCPs and track follow-up.

Coordinate referrals to specialty behavioral health and community resources.

Address social determinants of health, including transportation, housing, food insecurity, financial barriers, and access to care.

Collaborate with PCPs, psychiatric consultants, Care Management, and other members of the care team.

Maintain accurate and timely documentation of assessments, outreach, interventions, referrals, and care plans.

Support CoCM reporting, quality initiatives, and compliance requirements.

Participate in team meetings, case reviews, training, and continuous improvement initiatives.

Maintain patient confidentiality and comply with HIPAA and organizational policies.

Master’s Degree in Social Work (MSW) from a CSWE-accredited program.

Current social work licensure/credentialing appropriate to the state of practice.

Prior professional experience in behavioral health is required.

Experience working with depression, anxiety, and other behavioral health conditions.

Knowledge of behavioral health assessment, psychosocial interventions, crisis/safety assessment, and care coordination.

Strong communication, documentation, organization, and patient engagement skills.

Ability to manage a patient population through structured registry management and systematic follow-up.

Ability to work effectively within an interdisciplinary healthcare team.

Experience in CoCM, integrated behavioral health, primary care, or community mental health.

Experience with measurement-based behavioral healthcare.

Experience using PHQ-9, GAD-7, and other validated assessment tools.

Experience providing brief behavioral health interventions.

Experience working with primary care and psychiatric providers.
-
Experience with EHR and care management platforms.

Timely engagement and follow-up of assigned patients.

Accurate and current CoCM registry management.
-
Completion of assessments and reassessments within established timeframes.

Consistent psychiatric case review and follow-through on recommendations.

Appropriate identification and escalation of clinical or safety concerns.

Complete, accurate, and timely documentation.

Demonstrated progress toward patient-centered behavioral health goals.

Achievement of established productivity, quality, and patient engagement expectations.
