PHYSICAL THERAPIST (PT) – PERMANENT (SNF)

Location: CA 95695
Start Date: ASAP (Traveler in place: Tayla Henderson, PT – started 02/02/2026)
Schedule: Permanent position

Compensation:

$60 – $65 per hour

Position Summary & Responsibilities (SNF)

Evaluate and treat SNF residents
Document care and ensure Medicare/PDPM compliance
Collaborate with interdisciplinary team
Maintain productivity and quality standards

Requirements

Active Physical Therapist (PT) license
SNF or post-acute rehab experience required
Knowledge of Medicare/PDPM and documentation standards
Strong communication and teamwork skills
