Job Summary
We are seeking a compassionate and dedicated Polish-speaking Home Health Aide (HHA), Personal Care Assistant (PCA), or caregiver to provide exceptional in-home care for clients in need of support with daily activities. This role is vital in promoting independence, dignity, and quality of life for individuals across various age groups, including seniors, individuals with disabilities, and those receiving hospice or palliative care. The ideal candidate will be energetic, detail-oriented, and committed to delivering personalized patient care in accordance with home healthcare regulations. Fluency in Polish is essential to effectively communicate and build trust with clients and their families.
Duties
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support
- Monitor patient health by observing vital signs, documenting medical information, and reporting changes to healthcare professionals
- Support medication administration following prescribed protocols while maintaining strict adherence to HIPAA and patient confidentiality standards
- Operate Electronic Health Record (EHR) systems to accurately record patient data, treatment notes, and care plans
- Provide specialized dementia and Alzheimer’s care, including memory care techniques and end-of-life support
- Assist with catheter care, wound management, and other medical procedures as trained
- Facilitate safe patient transfers using methods such as Hoyer lifts or transfer belts while ensuring proper body mechanics and safety protocols
Requirements
- Proven experience working as a home health aide, PCA, or caregiver in assisted living facilities, nursing homes, or home healthcare settings
- Fluency in Polish with excellent communication skills to effectively interact with clients and their families
- Knowledge of medical terminology, vital signs measurement, medication administration, and basic life support (CPR/First Aid)
- Familiarity with home health care regulations and documentation requirements within EMR/EHR systems
- Experience working with individuals with developmental disabilities, dementia, geriatrics, or hospice needs is highly preferred
- Ability to perform heavy lifting and physical tasks safely; experience with patient transfer methods such as Hoyer lifts is advantageous
- Strong organizational skills for maintaining accurate medical records and ensuring compliance with privacy standards like HIPAA
