Home Health Registered Nurse (RN)
2+ Years Experience | Service Area: 60013, 61063, 60152 (Crystal Lake, Roscoe, Marengo & surrounding communities)Dedicated Home Health RN with over 2 years of experience delivering skilled nursing care across designated territories including ZIP codes 60013, 61063, and 60152. Experienced in managing diverse patient populations in home settings, ensuring continuity of care, and reducing hospital readmissions.
Key Responsibilities:
- Provide skilled nursing visits including SOC, ROC, Recertification, routine, and discharge across assigned ZIP code territories
- Perform comprehensive assessments and develop individualized Plans of Care based on physician orders
- Manage patients with complex conditions such as CHF, COPD, diabetes, post-hospitalization recovery, and mobility limitations
- Deliver hands-on care including wound care, PICC line management, IV therapy, Foley catheter care, and injections
- Monitor vital signs, lab values, and overall patient status; promptly report changes to physician
- Educate patients and caregivers on disease processes, medication management, and safety/fall prevention
- Complete accurate and timely OASIS documentation and maintain compliance with Medicare guidelines
- Coordinate care with interdisciplinary team including PT, OT, ST, and HHA services
