Position Overview (Certified Occupational Therapist Assistant – COTA)
Heritage Grove Estates is seeking motivated and compassionate Certified Occupational Therapist Assistants (COTAs) to join our in-house rehabilitation team for Full-Time and PRN opportunities. In this role, you will work under the supervision of a licensed Occupational Therapist to provide individualized therapy services that help residents regain independence, improve function, and enhance overall quality of life.
As a valued member of our interdisciplinary care team, you will support residents through therapeutic interventions focused on activities of daily living, mobility, safety, and functional independence within our skilled nursing environment.

Key Responsibilities
Provide occupational therapy services under the direction and supervision of a licensed Occupational Therapist
Implement individualized treatment plans designed to support resident goals and maximize functional independence
Assist residents with activities of daily living (ADLs), including dressing, bathing, grooming, mobility, and adaptive techniques
Monitor resident progress and communicate updates or changes in condition to the supervising Occupational Therapist
Accurately document treatments, progress notes, and outcomes in accordance with facility policies and regulatory standards
Collaborate with nursing staff, therapy team members, physicians, and interdisciplinary departments to ensure coordinated care
Educate residents and caregivers regarding therapeutic exercises, adaptive equipment, and safety strategies
Maintain a clean, safe, and organized treatment environment
Ensure compliance with ethical standards, facility policies, state regulations, and infection control procedures
Support a positive rehabilitation experience focused on resident-centered care and successful outcomes
Qualifications
Current Certified Occupational Therapist Assistant (COTA) certification and active state licensure required
Previous skilled nursing facility (SNF) or long-term care experience preferred
Strong communication and interpersonal skills
Ability to work collaboratively within an interdisciplinary team environment
Strong organizational and documentation skills
Commitment to delivering compassionate, high-quality resident care
Ability to adapt to changing resident needs and clinical priorities
