Position Overview
seeking an experienced Travel Social Worker for a hospital-based Behavioral Health assignment. The Social Worker will provide clinical social work services in an acute hospital environment, supporting patients through assessment, care coordination, and behavioral health needs.
Requirements
- Minimum 2 years of Social Work experience
- Acute hospital experience required
- Active Georgia LCSW required
- Prior travel experience required
- Strong hospital-based behavioral health experience
- Ability to work independently in a fast-paced acute-care environment
Key Responsibilities
- Provide clinical social work services to hospitalized patients
- Complete psychosocial and behavioral health assessments
- Identify patient needs and develop appropriate intervention plans
- Provide counseling, support, and crisis intervention as appropriate
- Coordinate care with physicians, nurses, case managers, and other disciplines
- Connect patients and families with appropriate community and post-acute resources
- Assist with discharge planning and transitions of care
- Advocate for patients and support their emotional and social needs
- Maintain accurate and timely clinical documentation
Experience:
- Social work: 1 year (Required)
- Acute hospital: 1 year (Required)
- Prior travel: 1 year (Required)
License/Certification:
- Active Georgia LCSW (Required)
