The Patient Safety Manager Registered Nurse plays a vital role in advancing the St. Cloud VA Health Care System's commitment to safe, high-quality, Veteran-centered care. This position offers the opportunity to influence facility-wide patient safety practices, collaborate with leaders and frontline teams, and strengthen systems that prevent harm and improve outcomes. Through data analysis, education, consultation, and process improvement, the Patient Safety Manager helps build a culture where staff are empowered, risks are identified early, and Veterans receive care in an environment focused on reliability, accountability, and continuous improvement.
Duties include, but are not limited to:
- Lead facility-wide patient safety programming, including Root Cause Analysis, Healthcare Failure Modes and Effects Analysis, proactive risk assessments, alerts, advisories, recalls, and National Patient Safety Goal activities.
- Review electronic patient event reports to ensure information is accurate, complete, and actionable for safety review and improvement efforts.
- Coordinate and support Root Cause Analysis activities by classifying events, identifying team members, advising RCA teams, ensuring technical requirements are met, and tracking follow-up actions and outcome measures.
- Analyze patient-reported events, near misses, complex safety data, and emerging trends to identify risks, recommend corrective actions, and support system-level improvements.
- Serve as the facility point of contact for Patient Safety Alerts and Advisories, National Center for Patient Safety concerns, and VHA patient safety guidance.
- Develop, coordinate, and deliver patient safety education, new employee orientation, training programs, and educational materials that promote a strong culture of safety.
- Provide consultation and guidance to staff and leaders on patient safety standards, requirements, policies, procedures, and improvement strategies.
- Collaborate across committees, forums, rounds, and review processes to strengthen compliance with patient safety standards, evaluate environmental risks, and support action plans that improve care delivery.
