Educational and Experience Requirement
Bachelor’s degree in related field. Minimum Five years of experience in healthcare, community outreach, patient services, or related field. Community Health Worker certification preferred. Bilingual – Fluent in Spanish preferred. Community Health Worker (CHW) Certification preferred or required within 12 months of hire (if applicable) as well as CPR/BLS certification. A combination of education, experience, and training that would produce the required knowledge and abilities could be considered.
Nature & Purpose of Position
Performs highly complex administrative and supervisory program work for the department of Primary Care and Clinical Medicine. Serves as a key member of the Mobile Health Unit (MHU) team, supporting the delivery of preventive health services through patient navigation, outreach, and care coordination. Responsible for facilitating access to care by assisting patients with eligibility determination, reducing barriers to services, coordinating referrals, and supporting engagement in preventive health programs. Operates in community-based settings and is essential to ensuring compliance with grant requirements, achieving program performance measures, and improving access to healthcare services for underserved populations.

Primary Responsibilities
Conducts eligibility screening and assist patients with enrollment in applicable health programs and services. Coordinates patient referrals to clinical providers, diagnostic services, and community resources. Provides follow-up support to ensure continuity of care and completion of recommended services. Maintains accurate documentation of patient interactions in accordance with institutional and grant requirements. Assists with appointment scheduling, reminders, and patient communication to support service utilization. Plans and implements outreach activities to promote awareness of Mobile Health Unit services. Identifies and engage underserved populations, including individuals with limited access to preventive health services. Establishes and maintains relationships with community partners, host sites, and local organizations. Supports development and implementation of outreach and engagement strategies aligned with program goals. Participates in community events, screenings, and educational activities as assigned. Collects and reports program data related to patient navigation, outreach, and service utilization. Ensures compliance with applicable policies, including Health Insurance Portability and Accountability Act (HIPAA), institutional procedures, and grant requirements. Participates in required training, program evaluation, and quality improvement activities. Assists in the implementation of program workflows and operational plans. Supports grant reporting and audit readiness activities as needed. Identifies barriers to care, including social determinants of health (e.g., transportation, financial, access issues). Provides information and referrals to community and social service resources. Advocates on behalf of patients to support timely access to services. Promotes responsive and patient-centered engagement practices. Performs other related duties as assigned.

This position is located in Conroe, Tx at the Medical School facility. Will require travel to locations in the eastern region of Texas.
