RN Registered Nurse - Hospice and Palliative Care - Home Health Visiting

Job Summary:
White Glove is seeking a dedicated and compassionate RN Registered Nurse to join our Hospice and Palliative Care team in Manhattan. As a RN Registered Nurse, you will provide high-quality home health visiting hospice care to patients and their families, working closely with our interdisciplinary team to ensure seamless care coordination.
Responsibilities:
Provide palliative and hospice care services to patients in their homes, focusing on symptom management, pain control, and emotional support
Conduct initial and ongoing assessments to identify patient needs and develop individualized care plans
Collaborate with patients, families, and interdisciplinary team members to provide comprehensive care
Educate patients and families on disease management, treatment options, and available resources
Maintain accurate and detailed records of patient care and progress
Participate in team meetings and stay informed about new developments in hospice and palliative care
Requirements:
Associate's degree in Nursing (ADN) or equivalent
2+ years of experience as a RN Registered Nurse, with a focus on hospice and palliative care
Current New York State RN registration
Active certification in Basic Life Support (BLS)
