Licensed Practical Nurse (LPN) – Long-Term Care | Night Shift| Day Shift
Location: Middlebury, VT
Schedule: 3*12-hour Nights | 5*8 Days Shift
Every other weekend required
Contract Length: Up to 32 weeks
About the Facility:
The center supports a collaborative work environment with strong LNA/CNA staffing, administrative support, and modern EPIC charting.
Position Overview:
We are seeking an experienced LPN to join the long-term care team. The ideal candidate will bring at least 1 year of recent LTC experience and demonstrate strong clinical judgment, communication skills, and commitment to excellent patient care.
Responsibilities:
- Provide direct resident care to a population primarily ages 60+
- Maintain accurate documentation using EPIC
- Collaborate with support staff including LNAs/CNAs and unit leadership
- Float as needed to Memory Care and other designated units
- Support daily care routines and contribute to a positive care environment
- Participate in a rotation that may include holidays and varying schedules
Shift & Staffing:
- Strong auxiliary support with 4 CNAs per shift
- Unit Secretary on day shifts
Requirements:
- Active LPN license
- Minimum 1 year of LTC experience
- Recent EPIC EMR experience (within last 2 years) – required
- Ability to work holidays as needed
- Prior travel experience preferred
