Blue Talent Development Agency is seeking a qualified and dependable Designated Manager to provide program oversight, compliance support, referral coordination, and community outreach for services provided to adults with physical and mental health disabilities.This is a part-time, contract/hourly position responsible for supporting the overall programmatic implementation and compliance with Minnesota DHS Chapter 245D licensing requirements. The Designated Manager will also help develop professional referral relationships with case managers, county agencies, healthcare professionals, and community organizations to connect eligible individuals with Blue Talent Development Agency services.
Key ResponsibilitiesProgram Management & 245D Compliance
- Maintain a current understanding of applicable Minnesota DHS 245D licensing requirements and support ongoing program compliance.
- Ensure the duties of the Designated Coordinator are fulfilled according to applicable 245D requirements.
- Provide program oversight and support consistent implementation of policies, procedures, and service standards.
- Ensure required internal reviews of incidents involving alleged or suspected maltreatment are completed in accordance with applicable requirements.
- Ensure the rights, dignity, safety, privacy, and choices of each person receiving services are protected.
- Evaluate satisfaction of individuals receiving services, legal representatives when applicable, and case managers.
- Maintain accurate program documentation and records required for compliance and service delivery.
- Support staff in maintaining compliance with agency policies, procedures, and applicable licensing standards.
Referral & Case Management Coordination
- Build and maintain professional relationships with case managers, county workers, social workers, healthcare professionals, and referral partners.
- Respond to referral inquiries and coordinate the intake and referral process.
- Follow up on referrals, assessments, authorizations, admissions, and next steps.
- Coordinate with internal staff to ensure timely and appropriate follow-up.
- Maintain professional communication and accurate referral documentation.
Community Outreach & Referral Development
- Build relationships with community agencies, healthcare providers, counties, and organizations serving adults with disabilities.
- Promote Blue Talent Development Agency’s services, eligibility, and referral process.
- Conduct outreach to develop appropriate referral partnerships.
- Maintain professional relationships with referral sources and community partners.
- Represent the agency professionally at meetings, networking events, and community activities.
Minimum Qualifications
Candidates must meet the applicable Minnesota 245D Designated Manager education, training, direct-care, and supervisory experience requirements.
Candidates may qualify through one of the following pathways:
Option 1 – Bachelor's Degree
- Bachelor's degree in a field related to human services; and
- Three years of full-time supervisory experience providing direct-care services to persons with disabilities or persons age 65 or older.
Option 2 – Associate Degree
- Associate degree in a field related to human services; and
- Three years of full-time supervisory experience providing direct-care services to persons with disabilities or persons age 65 or older.
Option 3 – Human Services Diploma
- Diploma in a field related to human services from an accredited postsecondary institution;
- Three years of full-time work experience providing direct-care services to persons with disabilities or persons age 65 or older; and
- Three years of supervisory experience.
Option 4 – Education & Training
- At least 50 hours of education and training related to human services and disabilities; and
- Four years of full-time work experience providing direct-care services to persons with disabilities or persons age 65 or older under the supervision of a staff person who meets the applicable qualification requirements.
Candidates must be able to demonstrate that their education and experience satisfy the applicable Minnesota 245D requirements.
Preferred Qualifications
- Experience working in a Minnesota DHS 245D-licensed human services program.
- Knowledge of 245D requirements and person-centered service delivery.
- Experience working with adults with disabilities.
- Experience communicating with case managers, counties, social service agencies, healthcare providers, or other referral sources.
- Experience with referrals, intake, community outreach, or relationship development.
- Strong communication, networking, organization, and follow-up skills.
- Strong understanding of person-centered practices and individual rights.
- Ability to work independently and manage multiple responsibilities.
Pay: $23.50 - $24.50 per hour
