Position Overview:
Provides services to clients in accordance with the Guam Nurse Practice Act, Health Services of the Pacific policy, and accepted professional standards of practice. The Nurse Practitioner provides nursing services utilizing a comprehensive base of nursing theory and nursing process, and communicates/documents observations and assessments. The Nurse Practitioner will provide care for the presenting patient from history taking, clinical assessment, diagnosis, treatment and evaluation of their care. Safe, clinical decision-making and expert care for patients will be demonstrated.
Qualifications:
1. Graduate of an accredited school of professional nursing. BSN, PHN preferred.
2. Graduate of an accredited Nurse Practitioner Program.
3. Current license as a Registered Nurse in the Territory of Guam.
4. CPR certified.
5. Current driver's license, good driving record, and reliable transportation.
6. Strong communication and interpersonal skills.
7. Computer Skills: Microsoft Office (Word, Excel, PowerPoint, Outlook)
Essential Job Functions:
1. Assumes responsibility and accountability for the practice of professional nursing in accordance with the Guam Nurse Practice Act and standards for home health nursing.
- Performs comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.
- Develops and/or follows an individualized Plan of Care that incorporates analysis of data and current scientific findings.
- Provides skilled interventions aimed at achieving realistic client outcomes within a specified time period.
- Reports changes and information necessary to modify and update the care plan to reflect progress towards goals (outcomes).
2. Consistently demonstrates competency with technical nursing skills according to personal and legal scope of practice.
- Assessment skills as applied to the client, family/support system, and environment.
- Teaching skills according to the client's needs and within the context of the client's environment, value system, and physical/emotional status.
- Medication administrations.
3. Practices nursing with respect for individual, cultural, and spiritual differences.
4. Communicates effectively and in accordance with agency requirements.
- Reports significant changes in client status to the physician and other members of the team in a timely manner consistent with client needs.
- Maintains client records showing systematic assessment, planning intervention, and evaluation.
- Documents legibly and according to agency documentation guidelines and standards.
5. Initiates actions to improve the quality of patient care. Takes appropriate actions independently and responsively in home care situations. Initiates referrals as needed to other health team.

9. Supervises applicable home care team members as delegated by Nursing Supervisor or Clinical Services Manager.
10. Performs job duties in accordance with agency policies, procedures, and professional and community standards.
- Maintains confidentiality in all aspects of the job. Does not reveal information from client records to others, except as identified in agency policy.
- Secures written confidential documents in a manner that prevents unauthorized release.
- Participates in the development, implementation, and evaluation of the Quality Improvement Program and activities.
- Provides care in accordance with state, federal, and JCAHO regulations.
11. Maintain complete and detailed records of patients’ health care plans and prognoses.
12. Educate patients about self-management of acute or chronic illnesses, tailoring instructions to patients’ individual circumstances.
13. Counsel patients about drug regimens and possible side effects or interactions with other substances such as food supplements, over-the-counter medications and herbal remedies.
14. Analyze and interpret patients’ histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnoses.
15. Diagnose or treat acute health care problems, complex, unstable, comorbid, episodic or emergency conditions in collaboration with other health care providers as necessary.
16. Develop treatment plans based on scientific rationale, standards of care, and professional practice guidelines.
17. Perform examinations and primary care procedures.
18. Provide patients with information needed to promote health, reduce risk factors, or prevent disease or disability.
19. Maintain current knowledge of state legal regulations for nurse practitioner practice.
20. Performs other duties as assigned.
