What You’ll Do
- Conduct comprehensive in-home nursing assessments
- Develop detailed, individualized care/service plans
- Perform routine supervisory visits and reassessments
- Evaluate client condition, safety, and functional status
- Supervise and document caregiver (PCA) performance
- Collaborate with physicians and care teams as needed
- Provide education and guidance to clients and families
- Complete thorough, compliant documentation (this is a major part of the role)
What We’re Looking For
- Active South Carolina RN license
- 3+ years of nursing experience (home care experience a plus)
- Strong documentation and assessment skills (required)
- Home care, case management, or clinical experience is a plus
- Comfortable using electronic documentation systems
- Reliable transportation for local travel
Important Note About This Role
This position is heavily documentation-based.

Availability
- PRN / As-needed
- Visits typically assigned 1–3 days in advance
