About the Role:
As a Social Services Coordinator, you will be responsible for coordinating and providing social services to our residents, including:
Conducting resident assessments and developing individualized care plans
Coordinating with healthcare providers, family members, and other stakeholders to ensure seamless care transitions
Providing emotional support and counseling to residents and their families
Assisting with daily living activities, such as mobility and transfer assistance
Maintaining accurate and thorough records of resident interactions and care provided
Responsibilities:
Develop and maintain relationships with residents, family members, and healthcare providers
Coordinate and facilitate resident activities, such as outings, events, and entertainment
Identify and report any changes in resident condition or needs to the healthcare team
Participate in inter-disciplinary care conferences to ensure comprehensive care planning
Maintain a safe and clean environment for residents, visitors, and staff
