Job Overview
We are seeking a compassionate and dedicated Home Health Aide to provide exceptional in-home care to clients across diverse needs. This vital role involves assisting individuals with daily activities, ensuring their comfort, safety, and well-being. As a Home Health Aide, you will be a trusted partner in promoting independence and enhancing quality of life for our clients, many of whom are elderly or living with disabilities. Duties
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and observe patient health status, documenting vital signs and changes in condition accurately using Electronic Medical Record (EMR) systems
- Support medication administration following prescribed protocols while maintaining strict adherence to home healthcare regulations and HIPAA confidentiality standards
- Provide specialized care such as Alzheimer's and dementia care, memory care, hospice support, and end-of-life assistance with compassion and professionalism
- Operate patient transfer methods safely, including Hoyer lifts and other mobility aids, ensuring patient safety during transfers
- Perform light housekeeping duties such as cleaning, laundry, meal preparation, and maintaining a safe environment for clients
- Communicate effectively with clients and their families to ensure personalized care plans are followed and needs are met promptly
Qualifications
- Proven experience working as a Home Health Aide or in assisted living, nursing home, or long-term care settings
- Knowledge of medical terminology, catheter care, vital signs monitoring, and basic life support (BLS) including CPR certification
- Experience working with individuals with developmental disabilities, geriatrics, or those requiring memory care or hospice services
- Familiarity with EMR/EHR systems for accurate medical documentation and record keeping
- Ability to lift heavy objects safely and perform patient transfers using appropriate techniques and equipment such as Hoyer lifts
- Strong communication skills for effective client interaction and team collaboration
- Valid certification in first aid and CPR; additional certifications related to dementia or Alzheimer's care are a plus
- Understanding of home health care assistance standards and home & community care experience is highly desirable
