The RN Case Manager is responsible for planning and managing the pediatric nursing care of an assigned caseload of patients primarily in the home care setting. In collaboration with the interdisciplinary team, the pediatric RN Case Manager ensures that patient and family member directed goals are achieved using a holistic, patient-centered approach. The role includes providing education, facilitating communication, and coordinating care across various sites. Flexibility working across patient site locations and care settings are required.
Position Details
- Job Location: Home care and hospital settings, predominantly in Duval County and surrounding counties
- Days/Hours: Monday - Friday 8:30am - 5:00pm; usually one weekend every six-weeks; sometimes 1-2 nights per week availability flex scheduling, depending on child availability and needs.
- Compensation: $74,000-$78,000 annual, plus paid driving mileage; full benefits package including 23 days annual PTO
Job Responsibilities
The RNCM uses critical thinking skills making decisions regarding patient care including knowledge and integration of relevant standards, resources, and data.
- Performs the initial assessment of the patient’s physical, functional, psychosocial and environmental needs and activities of daily living and identification of risks and interventions on admission.
- Performs initial assessment of family/caregiver needs and identification of risks and interventions on admission of the patient.
- Develops the initial POC in coordination with the IDG. Recommends to the IDG modifications to the POC based on patient/family/caregiver response to provided care and services.
- Implements the individualized POC with a focus on palliative care.
- Performs care coordination and appropriate referral and follow-up based upon the ongoing assessment of the patient and family/caregiver needs.
- Facilitates the transfer to another program of Community PedsCare when appropriate and necessary.
- Communicates with the Associate Medical Director and/or attending physician, if applicable, hospice employees and other health care providers to insure coordination and continuity of care.
- Routinely assists in the provision of after-hours on-call coverage.
Required Licenses/Certifications:
- State of Florida RN licensure
- Florida driver’s license with an acceptable driving record.
Education/Experience:
Any combination of education and experience that would provide required skill and knowledge for successful performance would be qualifying. Typical qualifications would be equivalent to:
- Completion of a nursing program from an accredited school of nursing, two (2) year Associate’s Degree or Bachelor’s Degree in Nursing.
- Two (2) years Registered Nurse experience within the past five years, or
- Two (2) years of pediatric experience, preferably obtained while working at a pediatric hospital.
Pay: $74,000.00 - $78,000.00 per year
