POSITION TITLE: Veteran Directed Care Counselor

THE ROLE DEFINED: A Veteran Directed Care Counselor helps Veterans assess their needs, create a spending plan and navigate self-direction process. Reporting to the Supervisor of MyCIL’s PACE & Veteran Directed Care Programs, the Veteran Directed Care Counselor will assist the Supervisor of MyCIL‘s PACE & Veteran Directed Care Programs with various administrative duties. In addition, collaborate with partners at Wilkes-Barre VA Medical Center.
Estimated Percent of Time: 75%
- Correspondence with referrals and initial intake.
- Review and explain Veteran Directed Care program.
- Assist Veterans and caregivers with the completion of program enrollment paperwork.
- Conduct in home assessments and complete assessment tool to determine Veterans’ level of assistance.
- Development of spending plan.
- Plan of care development.
Estimated Percent of Time: 20%
- Financial management services, including payroll for caregivers and payments for approved goods and services.
- Ongoing monitoring and documentation
Estimated Percent of Time: 5%
- Other duties as assigned.
DESIRED EXPERIENCE AND SKILLS:
- High school diploma or bachelor’s degree in human services or related field required.
- Minimum of 2 years’ experience in social services is preferred.
- Ability to familiarize self with Veteran Directed Care operating procedures and requirements.
- Ability to assess level of care and identify needs.
- Demonstrate ability to create a comprehensive plan of care.
- The successful candidate will be comfortable working on a computer and be proficient in MS Word and Excel.
- Excellent communication skills both oral and written.
- Must have strong organizational and analytical skills.
- Proof of a Registered Vehicle for use.
- Valid Pennsylvania Driver’s License required.
- Proof of Auto Insurance required.
