Overview
Kaleidoscope, INC. is seeking compassionate, dependable, and dedicated Community Living Supports (CLS) Contractors to provide person-centered support to individuals with intellectual and/or physical disabilities.
This is a contract position providing CLS services based on the individual’s approved service plan and needs. Contractors will work directly with individuals to promote independence, community involvement, and meaningful daily activities..
Duties
- Provide person-centered support according to the individual’s approved service plan.
- Assist with daily living activities and personal care as needed.
- Support individuals with community outings, appointments, recreational activities, and social opportunities.
- Encourage independence and development of daily living and community skills.
- Provide support in a safe, respectful, and positive manner.
- Follow all applicable service requirements, individual service plans, and Kaleidoscope policies and procedures.
- Complete required service documentation accurately and timely.
- Communicate appropriately with the individual, family/caregiver, case manager, and Kaleidoscope staff.
- Report any concerns, incidents, changes in condition, or other issues as required.
Qualifications
- Experience working with individuals with intellectual and/or developmental disabilities is preferred.
- Must be dependable, compassionate, patient, and responsible.
- Must be able to work independently while maintaining communication with the Kaleidoscope team.
- Must be able to follow the individual’s service plan and required documentation procedures.
- Must successfully complete required background checks and training.
- CPR/First Aid certification is preferred or must be obtained as required.
- Reliable transportation and a valid driver’s license may be required depending on the individual’s needs.
Contract Position
This is an independent contract position. Contractors are responsible for providing services in accordance with the individual’s approved service plan and applicable program requirements. Compensation and scheduling will be discussed during the contracting process.
