Schedule Details/Additional Information:
- Full-time and part-time openings
- Full time: 8a - 8p/ 3 days per week / every 3rd to 4th weekend
- Par time: 8a-8p / 2 days per week/ every 3rd to 4th weekend
We’re Looking For:
- Completion of an accredited or approved program in Nursing.
- Registered Nurse license issued by the state in which the team member practices
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA).
- Required to have a valid driver’s license issued by the Division of Motor Vehicles and acceptable vehicle operator record
- Pediatric Advanced Life Support (PALS) issued by the American Heart Association (AHA) preferred
- Minimum of two-years of recent nursing experience in an acute bedside setting or emergency nursing
- Pediatric experience strongly preferred within the last 3 years
What You’ll Do:
- Conduct comprehensive in-home patient assessments and develop individualized care plans using the nursing process.
- Monitor patient conditions, recognize changes early, and adjust care plans or escalate care as needed.
- Collaborate with patients, families, and interdisciplinary teams to coordinate safe, patient-centered home care.
- Apply evidence-based practice, nursing standards, and regulatory requirements to deliver high-quality care.
- Lead and communicate effectively with care teams, including appropriate delegation to support personnel.
- Promote patient and caregiver safety by identifying risks, preventing harm, and reporting safety concerns promptly.
- Support quality improvement, professional development, and age-specific care delivery across the lifespan.
