Job Title: Attending Physician (MD/DO) – Post-Acute Care / Skilled Nursing Facilities (Kentucky)

Job Overview
We are seeking a dedicated and experienced Attending Physician (MD or DO) to provide high-quality medical leadership and clinical care to patients in skilled nursing facilities (SNFs) and related post-acute settings in Kentucky. This is a flexible, field-based role focused on attending physician services, care plan oversight, and collaborative practice with advanced practice providers.
The ideal candidate is self-motivated, comfortable working independently in facility settings, and passionate about delivering excellent care to vulnerable populations in the post-acute continuum.
Key Responsibilities
- Conduct attending physician visits 2 days per week (approximately 4–6 hours total)
- Provide expert consultation on the plan of care during required monthly interdisciplinary reviews
- Review and sign off on all physician orders each month
- Perform comprehensive medical evaluations, chronic disease management, and acute care as needed
- Collaborate closely with Nurse Practitioners, Physician Assistants, facility staff, and interdisciplinary teams to ensure coordinated, high-quality care
- Document clinical encounters thoroughly and timely in the EHR
- Order and interpret diagnostics and develop/oversee comprehensive care plans
- Communicate effectively with patients, families, caregivers, and facility leadership
- Adhere to evidence-based guidelines, organizational protocols, and all state/federal regulations
- Participate in case reviews, quality improvement initiatives, and team meetings as needed
Qualifications & Requirements
- Active Kentucky medical license (MD or DO)
- Current DEA registration
- Board certification (preferred)
- Current ACLS/BLS certification
- Valid driver’s license and reliable transportation
- CAQH number
- Negative TB test (or proof of clearance)
- Strong clinical judgment and excellent communication skills
- Experience in geriatrics, post-acute care, or skilled nursing facility settings is highly preferred
Compensation & Schedule
- Competitive RVU-based compensation (daytime rate $22/RVU, nighttime rate $24/RVU)
- Flexible schedule: primarily 2 days per week (4–6 hours total)
