About Us:
Care To You Health is a therapist-owned mobile outpatient physical and occupational therapy practice serving adults throughout Southern California. We bring one-on-one therapy directly to patients in residential care facilities and homes.
We're looking for an organized, dependable Care Coordinator to help keep our operations running smoothly. If you enjoy staying organized, solving problems, and helping people behind the scenes, we'd love to meet you.
Responsibilities:
- Coordinate new patient referrals
- Schedule evaluations and follow-up visits
- Communicate with physicians, facilities, patients, and therapists
- Obtain and track physician orders and required documentation
- Follow up on pending referrals and missing paperwork
- Maintain accurate patient records in our electronic medical record (EMR)
- Answer phone calls and respond to emails professionally
- Coordinate appointments with therapists and equipment vendors
- Assist with general administrative and operational tasks
- Help ensure documentation is complete before patients are scheduled
Qualifications Required:
- Excellent organizational skills
- Strong written and verbal communication
- Reliable and dependable
- Comfortable using computers and learning new software
- Ability to multitask and stay organized
- Friendly and professional phone etiquette
- High attention to detail
Preferred:
- Healthcare office experience
- Medical scheduling or referral coordination
- Experience with EMRs
- Knowledge of Medicare or insurance is a plus (not required)

- Flexible hours
- Monday–Friday
- Approximately 10–20 hours per week (may vary based on workload)
- Mostly daytime hours
Pay: $16.50 per hour
