Job Summary
We are seeking a Board-Certified Internal Medicine Physician with expertise in Value-Based Care (VBC) to join our team in The Villages. This role is designed for a clinician dedicated to patient-centered care, focusing on long-term health outcomes, preventative strategies, and cost-effective management of acute and chronic conditions. You will lead a multidisciplinary team to improve patient health metrics while operating within a performance-based reimbursement model.
Key Responsibilities
- Comprehensive Primary Care: Manage a diverse adult and geriatric patient population with a focus on chronic disease management (e.g., Diabetes, Hypertension, COPD).
- VBC Alignment: Practice according to Value-Based Care principles, focusing on quality metrics, patient satisfaction, and closing care gaps.
- Risk Adjustment & Coding: Utilize EMR systems for accurate documentation and coding in compliance with value-based programs, including HEDIS, CMS Star Ratings, and RAF/HCC coding.
- Collaborative Leadership: Coordinate with care teams, specialists, and ancillary providers to ensure seamless, integrated care transitions.
- Preventative Focus: Engage patients in shared decision-making, emphasizing lifestyle modifications, self-management, and preventative screenings.
- Quality Improvement: Participate in organizational initiatives to enhance clinical performance and cost efficiency.
Qualifications
- Education: Medical Degree (MD or DO) from an accredited institution.
- Certification: Board Certification in Internal Medicine (required).
- Licensure: Active, unrestricted license to practice medicine in the State of Florida.
- Experience: Minimum 3+ years in primary care; demonstrated experience in a Value-Based Care or Risk-Adjustment environment is highly preferred.
- Skills: Strong interpersonal skills to build long-term relationships with a senior-heavy demographic in The Villages.

Pay: $250,000.00 - $350,000.00 per year
