Abbey Hospice is a compassionate hospice organization dedicated to providing exceptional end-of-life care to patients and their families. We are seeking a caring and experienced LMSW or LCSW to join our interdisciplinary team. Duties
- Conduct psychosocial assessments of hospice patients and families.
- Provide emotional support, counseling and crisis intervention.
- Assist patients and families with advance directives, end-of-life planning.
- Connect patients and caregivers with community resources and support services.
- Collaborate with physicians, nurses, chaplains, and other team members to develop and implement care plans.
- Maintain accurate and timely documentation in compliance with regulatory requirements.
Qualifications
- Master's Degree in Social Work (MSW) from an accredited program.
- Current state social work license (LMSW, LCSW, or equivalent) preferred.
- Hospice, palliative care, home health, or healthcare experience preferred.
- Strong knowledge of behavioral health interventions including cognitive behavioral therapy (CBT), applied behavior analysis (ABA), and psychotherapy techniques.
- Excellent communication and interpersonal skills
- Valid driver's license and reliable transportation
