Community Health Navigator (3 Openings)
Remote with Local Travel Required | Full-Time | $39,000/year ($18.75/hour)
A.R. Mazzotta is partnering with a local healthcare organization to recruit three Community Health Navigators for an exciting new Rural Health Transformation (RHT) Program. This federally funded, five-year initiative is designed to improve healthcare access, quality, and outcomes in rural Connecticut communities by connecting HUSKY Health members with providers, services, and community resources.
These are fully remote positions that require regular local and regional travel. To best support the communities being served, candidates must reside in one of the following areas:
- Litchfield County, CT
- Windham County, CT
- New London County, CT
- We are ideally seeking one Community Health Navigator assigned to each region.
Position Summary
As a Community Health Navigator, you will work closely with Community Health Workers and community partners to support HUSKY Health members and their families. You will help individuals navigate healthcare systems, access community resources, and address barriers that impact their health and well-being. This role combines outreach, advocacy, education, and care coordination to improve health outcomes in rural communities.
What You'll Do
- Build positive, trusting relationships with HUSKY Health members and their families
- Connect members to healthcare providers, community services, and other available resources
- Assist members with identifying primary care providers and scheduling appointments
- Provide social support, education, and advocacy to help members navigate healthcare and social service systems
- Conduct outreach through community events, health fairs, and visits in various community setting
- Address Social Drivers of Health (SDOH) needs through resource coordination and referrals
- Follow up with members to ensure ongoing support and successful connections to services
- Collaborate with healthcare providers, community organizations, and internal team members to improve health outcomes
- Participate in meetings, trainings, and community engagement activities as required
What We're Looking For
- High school diploma or GED required; Associate degree in Social Services, Public Health, Allied Health, or a related field preferred
- Minimum of one year of work or volunteer experience helping others or supporting vulnerable populations
- Experience in community outreach, health education, social services, case management, or community-based organizations preferred
- Understanding of Social Drivers of Health (SDOH) and community resources preferred
- Strong communication, relationship-building, and problem-solving skills
- Basic computer proficiency and comfort learning new technology
- Bilingual English/Spanish skills are a plus
- Valid driver's license and reliable transportation required
- Must reside in Windham County, Litchfield County, or the New London County. Torrington area
Schedule
- Full-time, Monday through Friday
- Flexibility to work occasional evenings and weekends based on community events and program needs
Work Environment
This is a telecommuting position requiring a HIPAA-compliant home workspace and reliable high-speed internet access. While remote, the position requires frequent travel throughout the assigned community for outreach activities, meetings, trainings, and events.
