Job Overview
We are seeking a compassionate and motivated Physical Therapist (PT) to join our therapy team in a nursing home / skilled nursing facility. This position focuses on providing high-quality rehabilitative care to short-term rehab and long-term care residents, helping them improve mobility, strength, and overall functional independence.
Responsibilities
- Patient Evaluation: Assess patients' physical abilities and limitations through comprehensive evaluations to develop individualized treatment plans.
- Treatment Implementation: Provide therapeutic exercises, modalities, and rehabilitation techniques to improve patients' mobility, strength, and overall function.
- Documentation: Maintain accurate records of patient progress, treatment plans, and interventions using the SOAP (Subjective, Objective, Assessment, Plan) format. This included making sure you complete documentation accurately and in a timely manner (evaluations, daily notes, progress notes, discharges)
- Collaboration: Work with physicians, nursing staff, and other healthcare professionals to coordinate care and ensure quality treatment for patients. Provide skilled PT services in accordance with the physician orders. Monitor patient progress and adjust treatment plans as needed.
- Patient Education: Educate patients and their families about treatment plans, exercises, and strategies to manage their conditions effectively.
- Supervision: Oversee and assist physical therapy assistants (PTAs) in delivering care and ensure compliance with treatment protocols.
- Treat residents with a variety of diagnoses including post-surgical, orthopedic, neurological, and general weakness
- Ensure compliance with facility policies and state and federal regulations
Qualifications
- Current Physical Therapist license in Arizona is required
- Graduate of an accredited Physical Therapy program required
- Experience in a SNF or long-term care setting preferred (new graduates welcome to apply)
- Strong communication and time-management skills
- Ability to work independently and as part of a team
