Illumination Health + Home is seeking an experienced and mission-driven Director of Services & Care Transitions (RN) to provide strategic and clinical leadership across our Recuperative Care and Enhanced Care Management (ECM) programs.
This is a senior leadership opportunity for an accomplished Registered Nurse who is passionate about improving healthcare for vulnerable and underserved populations. The Director will lead clinical teams, oversee care management programs, strengthen care transitions, and help develop scalable models that integrate medical, behavioral health, and social services.
As a key member of the senior leadership team, you will partner closely with operations and program leaders to ensure high-quality, compliant, patient-centered care while advancing the growth and effectiveness of our programs.
What You'll Lead
Clinical Leadership & Quality
- Provide leadership and direct supervision to clinical staff within the Recuperative Care program, including RNs, LVNs, and medical assistants.
- Establish and maintain clinical standards, protocols, workflows, and evidence-based practices.
- Monitor clinical performance, quality measures, and client outcomes.
- Lead quality and performance improvement initiatives.
- Ensure compliance with applicable regulatory, licensing, and accreditation requirements.
- Build a culture of clinical excellence, accountability, collaboration, and compassionate care.
Enhanced Care Management & Program Development
- Provide leadership and oversight for Enhanced Care Management (ECM) programs and staff.
- Lead the continued development and expansion of Recuperative Care, ECM, and related care management services.
- Develop scalable care models integrating medical, behavioral health, case management, and social services.
- Standardize case management workflows, documentation practices, client engagement, and continuity-of-care processes.
- Develop strategies to improve client acuity management, service prioritization, and caseload effectiveness.
- Establish and monitor KPIs related to care management effectiveness, client outcomes, and service delivery.
- Use data and outcomes to identify opportunities for continuous improvement.
- Partner across CalAIM-related programs to strengthen integration and performance.
Care Transitions & Operational Partnership
- Partner closely with Operations to align clinical services with program capacity, workflows, and client needs.
- Strengthen transitions of care and coordination between healthcare providers, community programs, and internal services.
- Promote interdisciplinary collaboration among medical, behavioral health, and case management teams.
- Support efficient utilization of clinical resources while maintaining high standards of care.
- Participate in program planning, budgeting, resource allocation, and performance management.
Organizational Leadership
- Serve as a key member of the senior leadership team and contribute to organizational strategy, growth, and decision-making.
- Identify opportunities for program innovation, partnerships, and funding.
- Represent Illumination Health + Home with healthcare organizations, payers, community partners, and other stakeholders.
- Ensure programs remain culturally responsive, equitable, trauma-informed, and centered on the needs of the individuals we serve.
What We're Looking For
Required Qualifications
- Active Registered Nurse (RN) license in the State of California
- Bachelor of Science in Nursing (BSN)
- 5–7+ years of progressive healthcare leadership experience
- Demonstrated experience leading clinical teams and overseeing care delivery in complex healthcare environments
- Strong understanding of Enhanced Care Management (ECM), Medi-Cal populations, and care management models
- Experience overseeing interdisciplinary care coordination and case management
- Experience developing workflows for case management, documentation, client engagement, and continuity of care
- Experience serving vulnerable, underserved, or medically complex populations
- Strong leadership, communication, collaboration, and performance-management capabilities
Highly Preferred Experience
- Recuperative care, homeless services, community health, FQHC, or community-based healthcare experience
- Master's degree in Nursing, Public Health, Healthcare Administration, or related field
- Knowledge of CalAIM Community Supports
- Care transitions and discharge planning
- Utilization management
- Trauma-informed care
- Motivational interviewing and harm reduction
- Client-centered service delivery models

Pay: $150,000.00 - $170,000.00 per year
