We are seeking a detail-oriented and knowledgeable MDS Coordinator to join our team. This role is responsible for overseeing the accurate and timely completion of the Minimum Data Set (MDS) assessments in compliance with all state, federal, and facility guidelines. The MDS Coordinator works closely with the interdisciplinary team to ensure precise resident assessments, proper care planning, and accurate reimbursement.
Key Responsibilities:
- Complete, track, and submit MDS assessments within required timelines.
- Coordinate and lead interdisciplinary care plan meetings.
- Ensure accurate coding and documentation to support resident needs and reimbursement requirements.
- Monitor changes in resident status and initiate appropriate assessments.
- Maintain compliance with regulatory standards and participate in audits as needed.
- Collaborate with nursing, therapy, and administrative teams to support quality outcomes.
Qualifications:
- Current RN (preferred) or LPN license. Pay based on licensure and experience.
- Knowledge of (or willing to be trained on) MDS, RAI process, and skilled nursing regulations.
- Excellent organizational, communication, and assessment skills.
- Ability to work collaboratively and maintain accuracy in a fast-paced environment.
This position plays a vital role in resident care quality, regulatory compliance, and the financial health of the facility.
