Haven Link is seeking a compassionate and dedicated Registered Nurse (RN) to support individuals receiving services through the Home and Community-based Services (HCS) Host Home Program in Crystal City and surrounding areas.
Responsibilities:
- Conduct nursing assessments and health monitoring
- Develop and update nursing care plans
- Review physician orders and medications
- Coordinate medical appointments and follow-up care
- Train host home providers and caregivers
- Ensure compliance with HCS and HHSC regulations
- Maintain accurate nursing documentation
- Participate in interdisciplinary team meetings
Qualifications:
- Active Texas RN license in good standing
- 1 year of experience with HCS, IDD, or community-based services preferred
- Strong organizational and communication skills
- Reliable transportation for local travel

Pay: $35.00 - $36.91 per hour
