What You’ll Do
As an Occupational Therapist, you will play a key role in developing and implementing therapy plans that support patient independence and overall well-being. Responsibilities include:

Performing comprehensive assessments of patients’ functional abilities and home environments

Developing and implementing individualized treatment plans and therapy goals

Providing occupational therapy services as ordered by the physician and outlined in the plan of care

Educating patients, families, and caregivers on treatment plans and adaptive techniques

Teaching or retraining patients in daily living skills affected by illness, injury, or disability
-
Recommending and training patients in the use of adaptive equipment, splints, prosthetics, and orthotics as appropriate

Monitoring patient progress and adjusting treatment plans as needed

Maintaining accurate and timely clinical documentation

Communicating and collaborating with physicians, nurses, therapists, and other care team members

Supervising Certified Occupational Therapy Assistants (COTAs) in accordance with regulations and company policy

Identifying and coordinating referrals for additional home health services as needed
-
Participating in case conferences, quality improvement initiatives, and interdisciplinary care planning

Assisting with discharge planning to ensure continued patient success and safety
What We’re Looking For
-
Current Occupational Therapist (OT) license in the State of Wisconsin
-
Graduate of an ACOTE-accredited Occupational Therapy program
-
Eligible for or successful completion of the NBCOT certification examination

Minimum of 2 years of clinical experience preferred

Strong assessment, communication, and organizational skills

Ability to work independently while collaborating effectively with an interdisciplinary team

Commitment to providing compassionate, patient-centered care

Valid driver’s license with a reliable, insured vehicle
