WHAT WILL YOU DO?
We are looking to add to our professional team of PDMS-525 Service Coordinators. The PDMS Service Coordinator supports individuals and families who have chosen the Participant Directed and Managed Services (PDMS) model by facilitating access to self-directed, community-based services. This role empowers individuals to design and manage services that promote independence, dignity, and quality of life. Through personalized support, advocacy, and education, the PDMS Service Coordinator assists with planning, implementation, monitoring, and compliance of services under the He-M 525 regulations within the DD waiver.
RESPONSIBILIITES
- Assist individuals and families to advocate for themselves and advocate on behalf of individuals and/or family for services.
- Provide direct assistance to individuals/families as needed and as required by agency and state regulations.
- Facilitate service planning process and team meetings to assure meetings promote individual choice & control and address the needs of the individual/family.
- Develop a trusting relationship with individuals and families to foster open communication.
- Facilitate and develop annual service agreements and amendments for all individuals with community care waiver funded services.
- Monitor implementation of funded services, progress, and addressing any needed changes.
- Assure individuals have knowledge of application process for benefit entitlements which are necessary for funding and to assure that all annual Medicaid re-determinations are completed as required.
- Assess the individuals need for services and complete necessary requirements to assure the person is identified as in need of services in electronic record systems.
- Complete all required documentation and maintain an electronic record systems.
- Assist families in the development of services. This includes assisting in the selection of a service provider, that will meet the needs identified in Individual Service Agreement.
- Monitor services to ensure the health, safety, and basic needs of the individual are being met and to report any allegation of abuse, neglect, or exploitation as required by agency policy and state regulations.
- Work collaboratively within the agency departments as well as with the Bureau of Developmental Services—ensuring that timely and effective communication occurs related to the individual’s support needs
- Attend necessary statewide and agency wide meetings and trainings related to the roles and responsibilities of a service coordinator
- Represent the agency in a professional manner at all state and local meetings.
- Ensure continuity and quality of services.
QUALIFICATIONS
- Two years’ experience working in human services with individuals or families or related field or Associate/Bachelors Degree in Psychology or Social Work field.
- Experience in service coordination or case management, preferably with participant-directed or Medicaid waiver programs.
- Knowledge of PDMS and He-M 525 regulations, as well as employment-related responsibilities under participant direction.
- Strong interpersonal, organizational, and communication skills.
- Ability to manage multiple cases and deadlines while maintaining accurate documentation.
- Comfort navigating family dynamics and problem-solving service delivery challenges in a respectful, collaborative manner.
