Responsibilities
- Assess patients’ physical conditions by conducting thorough evaluations of physiology, anatomy, and medical history.
- Develop and implement individualized treatment plans that incorporate therapeutic exercises, trigger point therapy, and other physiotherapy techniques.
- Manage discharge planning processes to ensure seamless transitions from hospital or inpatient settings to home or community care.
- Document all patient interactions accurately following clinical documentation standards and electronic health records (EHR) management protocols.
- Collaborate closely with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate comprehensive care strategies.
- Provide patient education on rehabilitation exercises, injury prevention, and health maintenance tailored to geriatric, pediatric, neurological, orthopedic, or outpatient populations.
- Participate in outpatient clinic activities and home health visits to deliver consistent patient care across various environments.
Requirements
- Valid licensure as a licensed Physical Therapist with practicing in a physical therapy role.
- Demonstrated experience in acute care, inpatient hospital settings, outpatient clinics, or outpatient rehabilitation centers.
- Strong knowledge of physiology, anatomy, medical terminology, and clinical documentation standards.
- Proven ability to develop effective treatment plans for diverse populations including geriatrics, pediatrics, neurology patients, and those requiring orthopedics or trigger point therapy.
- Experience working with electronic health records (EHR) systems such as EMR systems for accurate documentation and communication.
- Excellent client communication skills with the ability to educate patients on therapy goals and progress.
- Familiarity with discharge planning processes and working within hospital or home & community care environments is preferred.
- Ability to work collaboratively with healthcare teams in inpatient or outpatient settings while maintaining high standards of patient care.

Pay: $77,200.00 - $112,000.00 per year
