Essential Duties and Responsibilities:
- Manage a mixed caseload that may include Housing Access Services participants and participants from other Catalyst programs such as ILS, SLS, Tailored Day, and other assigned service areas.
- Process referrals of clients from appropriate referring agencies.
- Expedite the intake and assessment process.
- Provide housing navigation services, including assessment of housing needs, support with housing applications, coordination of required documentation, and development of individualized housing stability plans.
- Coordinate affordable housing resources and assist participants in understanding eligibility requirements, application processes, documentation needs, and available community support.
- Provide lease education and support participants in understanding tenant rights, tenant responsibilities, reasonable accommodation processes, housing expectations, and steps needed to maintain successful tenancy.
- Support eviction-prevention and tenancy stabilization efforts by identifying housing-related concerns early, coordinating resources, documenting participant needs, and elevating concerns through appropriate channels.
- Effectively manage, develop and evaluate direct support staff.
- Assess client collateral data, information from intake interview, and conduct ecological studies of client's abilities and potential living and work environments.
- Develop placements for clients in work and/or living situations that best match the individual's strengths and availability.
- Participate in ongoing training and development, taking responsibility for keeping updated on changes in policies, procedures and protocol.
- Prescribe, implement, and supervise individual training and support programs.
- Project staffing needs and communicates those needs to the Regional Manager and/or his/her designee.
- Collaborate with and develop positive communications and relationships with individuals in cooperative agencies, employers, involved members of families and clients towards the achievement of service goals.
- Collaborate with landlords, housing authorities, Regional Centers, community partners, families, support teams, and internal program leadership to coordinate participant-centered housing access and stability supports.
- Maintain working knowledge of Title 17 Housing Access Services requirements and applicable program expectations to support compliant, high-quality service delivery.
- Partner with the Housing Manager for guidance on program standards, housing-related resources, and escalation needs, while program development, landlord recruitment, housing partnerships, and overall program oversight remain assigned to the Housing Manager.
- Participate in the hiring, training, evaluation, and separation of direct support staff.
- Complete in a timely manner all logs, reports and client evaluations as required.
- Review and assure all direct service staff’s documentation is accurate and complete.
- Evaluate and respond appropriately to client and personnel needs and issues, recognizing and taking appropriate action where needed and elevating concerns through chain of command as appropriate.
- Provide direct service as needed and required.
- Be part of the on-call rotation as needed and required.
- Travel to job fairs, client meetings, employer sites, and other locations as required, ensuring safety and professionalism while driving.
- Advocate and perform duties with a person-centered methodology.
- Other duties as required.
Minimum Qualifications:
- 18 years of age or older.
- Knowledge of American Sign Language (ASL) and/or Lengua de Señas Mexicana (LSM) highly preferred to strengthen accessibility and inclusive service delivery.
- Bachelor's degree or three years equivalent experience and/or educational equivalent in working with individuals with disabilities.
- Prior successful experience as a Catalyst Case Manager or comparable case management experience supporting individuals with intellectual and developmental disabilities, housing stability needs, or community-based services preferred for promotional-level consideration.
- English and Spanish proficiency required whenever operationally feasible to support effective communication with the diverse communities served.
- Working knowledge of Title 17 Housing Access Services requirements, affordable housing resources, housing navigation practices, tenant education, and housing stability planning preferred or required based on operational need.
- Since this position requires a great deal of interaction with people, well-developed interpersonal skills are critical.
- Demonstrated computer literacy (MS Office) and capability to maintain well-organized records and files.
- Ability to follow detailed, multi-step instructions is critical to this position.
- Ability to multi-task, work under pressure and manage time is also an essential component of this position.
- Demonstrated ability to effectively navigate and maintain professional boundaries as custodian of confidential information.
- Certification in an approved First Aid and CPR class; or successfully complete an approved First Aid and CPR class within 45 days of the date of hire.
- Insured, registered vehicle in safe working conditions, and proof of current CA driver’s license.
Position Requirements:
- Ability to travel within and throughout the counties in which services are provided a minimum of 75% of the time.
- Ability to travel in and throughout the state on a quarterly basis to attend meetings, training, etc.
- Successful completion of pre- employment drug screen and background check.
- Physical evaluations may be required dependent upon specific assigned duties or compliance requirements and may include a TB test and/or a lift and transfer test.
- Acceptable DMV record.
- Adjoin’s client case management platform requires employees to utilize a mobile smart device (i.e. cell phone/tablet) with reliable internet connectivity and data to complete certain essential job tasks. This position requires the employee to have a mobile smart device available to perform certain required essential functions, for which reasonable reimbursement is provided to the employee.
Work Requirements:
- This position requires the employee to regularly stand, walk, sit, talk and hear. Capable of writing, using a computer keyboard, telephone and related office equipment. Ability to lift 25lbs required. This position will normally spend long periods of time in a sitting position (80% of the day). They may use a computer, which would involve repetitive finger, hand and wrist motion (75% of the day) and require being able to see the computer screen (90% of the day). Other physical exertion could involve walking, climbing stairs, lifting, stretching, bending, etc.
- The Specialized Case Manager must be able to perform direct service as needed. Some of our clients are physically impaired and require assistance in and out of wheelchairs and other assistive devices. The employee must be able to perform such tasks on an urgent or emergency basis. Many of the jobs/activities in which our clients engage require physical exertion on the part of the Adjoin staff person (i.e., walking (75% of workday), climbing stairs (10% workday but on a frequent basis), lifting (10% of workday but on a frequent basis), stretching, bending, etc.). Those providing direct service must have the ability to talk, hear, and see to assist the client in his/her quest for independence.
- The employee is responsible for the safety and well-being of a disabled client they are providing service for. Must be able to react appropriately and quickly in the event of an emergency such as client elopement, a natural disaster, fire or other unforeseen circumstances. An appropriate reaction might be having to follow or help the client evacuate in the event of a fire or emergency which may involve ascending or descending stairs, walking a significant distance, or being able to summon emergency responders.

- The employee will experience normal office conditions with a well-lighted work area and minor noise from standard office equipment. In the community the employee will be working in community environments, employment sites and office environments. Potential exposure to noisy environments at job sites. Exposure to blood borne pathogens could occur. (Hepatitis B vaccines are offered to all employees at no cost to the employee.)
- The Case Manager is expected to perform duties assigned during regular working hours. As necessary, the Case Manager may be called upon to work in the evenings or on weekends. Overnight travel may be required in the performance of the Case Manager’s job duties. Unless otherwise stipulated, a normal workweek will be forty hours.
