Job Summary
We are looking for a Registered Nurse (RN) who provides skilled, patient-centered nursing services in home settings. Responsible for conducting comprehensive patient assessments, developing individualized plans of care, providing skilled nursing interventions, coordinating care with physicians and interdisciplinary team members, and educating patients and caregivers to promote optimal health outcomes, independence, and quality of life.
Key Responsibilities and Duties
- Conduct comprehensive initial and ongoing patient assessments, including physical, psychosocial, and environmental evaluations.
- Develop, implement, and update individualized plans of care based on physician orders, patient needs, and evidence-based nursing practice.
- Perform skilled nursing services including medication administration, wound care, catheter care, IV therapy, infusion therapy, ostomy care, disease management, and other physician-ordered treatments.
- Monitor patient condition, identify changes in clinical status, and promptly communicate significant findings to the physician and interdisciplinary care team.
- Complete medication reconciliation and educate patients and caregivers regarding medications, disease processes, treatment plans, infection prevention, and self-management techniques.
- Coordinate care with physicians, therapists, medical social workers, home health aides, and other healthcare professionals to ensure continuity of care.
- Complete all required documentation, including OASIS assessments, care plans, visit notes, recertifications, transfer summaries, and discharge documentation in accordance with agency, state, and federal requirements.
- Supervise Licensed Practical Nurses (LPNs), Home Health Aides (HHAs), and other assigned personnel to ensure services are provided safely and in accordance with the patient's plan of care.
- Participate in discharge planning and ensure appropriate transition of care when skilled nursing services are no longer required.
- Respond appropriately to emergency situations and implement interventions within the scope of nursing practice.
- Maintain compliance with all applicable federal, state, accreditation, and agency policies regarding patient care, documentation, infection control, and confidentiality.
- Maintain professional competency through continuing education, annual in-service training, and participation in agency quality improvement initiatives.
Education & Licensing
- Must be licensed as a Registered Nurse (RN) in the State of Arizona.
- Graduate of an accredited school of nursing.
Certifications
- Valid BLS certification (e.g., American Heart Association or American Red Cross).
Experience
- 1–2 years of home health nursing experience
Pay: $40.00 - $50.00 per hour
Work Location: In person
