Key Responsibilities:
✔ Provide high-quality, one-on-one skilled nursing care in patients' homes.
✔ Perform medication management, wound care, IV therapy, and patient education.
✔ RNs: Conduct OASIS assessments and develop individualized care plans.
✔ LPNs: Deliver skilled nursing care under RN supervision and collaborate on patient management.
✔ Work alongside a multidisciplinary healthcare team to ensure optimal patient outcomes.
