We are seeking a dedicated and compassionate Children’s Care Coordinator to join our team. Responsibilities
- Serve as the primary point of contact for children and their families, providing guidance and support throughout their care journey.
- Coordinate appointments, follow-up care, and communication between healthcare providers, families, and community resources.
- Maintain accurate documentation of medical records, appointments, and service plans in compliance with privacy regulations.
- Assist with administrative tasks such as scheduling, data entry, and managing medical office correspondence.
- Collaborate with medical professionals to ensure children’s needs are met efficiently and effectively.
- Provide educational information to families regarding treatment options, community resources, and health maintenance.
- Support the implementation of care plans by liaising with multidisciplinary teams to promote optimal outcomes.
Requirements
