Responsibilities
- Assess patients’ physical conditions through comprehensive evaluations, including mobility, strength, balance, and functional abilities.
- Develop and implement individualized treatment plans aligned with clinical best practices and patient goals.
- Deliver hands-on therapy interventions such as therapeutic exercises, manual therapy, trigger point therapy, and gait training within home settings.
- Coordinate discharge planning and ensure seamless transition of care by collaborating with physicians, nurses, caregivers, and other healthcare providers.
- Maintain accurate and timely clinical documentation in accordance with medical terminology standards and electronic health records (EHR/EMR systems).
- Educate patients and caregivers on injury prevention, activity modifications, and home exercise programs to support ongoing recovery.
- Conduct regular progress assessments to modify treatment plans as needed and ensure optimal patient outcomes.
Skills
- Strong knowledge of physiology, anatomy, orthopedics, neurology, geriatrics, pediatrics, and outpatient rehabilitation principles.
- Proficiency in clinical documentation standards and electronic health records (EHR/EMR systems) management.
- Excellent client communication skills to effectively educate and motivate patients throughout their rehabilitation journey.
- Experience with discharge planning processes and working with diverse populations in home health or outpatient settings.
- Ability to collaborate effectively with healthcare professionals across disciplines to develop comprehensive care plans.
- Familiarity with inpatient hospital experience or outpatient clinic experience is advantageous for understanding various care environments.
- Knowledge of medical terminology and physiotherapy techniques such as trigger point therapy enhances treatment delivery.
