We at Chaim Gitelis DO PC are a growing preventative healthcare company that partners with long-term care facilities to provide innovative healthcare solutions through Remote Patient Monitoring (RPM) and Chronic Care Management (CCM). We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team full-time and support residents within partner facilities.
Position Overview:
The LPN will play a vital role in bridging patient care with technology by monitoring vital signs, facilitating telehealth visits, and being a point of communication with residents enrolled in our RPM and CCM programs.
Responsibilities:
- Take and record vital signs for residents enrolled in the RPM program using company-provided equipment.
- Facilitate telehealth visits and calls between residents and Nurse Practitioners (NPs).
- Maintain accurate documentation of all patient interactions and vitals.
- Collaborate with NPs, facility staff, and our health care team to ensure seamless quality care.
Qualifications:
- Active LPN license in the state of TX.
- Strong clinical and documentation skills.
- Comfortable with telehealth platforms and medical technology (training provided).
- Excellent communication and interpersonal skills.
- Reliable, organized, and able to work independently within a facility setting.
Schedule:
- Full-time position
- Standard daytime hours with some flexibility based on facility needs
