Evaluate residents and develop individualized treatment plans
Help residents improve their ability to perform activities of daily living (ADLs)
Address deficits related to strength, coordination, cognition, safety awareness, and functional independence
Recommend adaptive equipment and environmental modifications
Collaborate with nursing, therapy, physicians, families, and interdisciplinary team members
Assist with discharge planning and safe transitions home
Educate residents and caregivers on techniques that improve safety and independence
Maintain timely and accurate documentation
Participate in care planning meetings and interdisciplinary discussions
Current Occupational Therapist license in the State of Colorado
Graduate of an accredited Occupational Therapy program
Excellent communication and interpersonal skills
Strong clinical reasoning and problem-solving abilities
Passion for helping older adults maximize independence and quality of life
Skilled Nursing Facility experience preferred, but not required
New graduates are encouraged to apply
$40–$50 per hour
