Job Title: Home Health RN
Assignment Length: 17 weeks (potential to Perm)
Location: Detroit, MI
Shift: 8 am-4:30 pm; and one Saturday every Month
Pay Rate: $54/hour
Responsibilities:
- Assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families.
- Patient care is delivered within organizational philosophy, policy, and standards of community health practice.
- Perform admission processes and case management responsibilities in accordance with the Home Care’s policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications.
Experience:
- 1 year Certified Home Care experience- a Must!!
- 1 year Case Management Experience.- a Must!!
- Ability to travel to cases as assigned.- a Must!!
Job Requirements:
- RN License in MI
- BLS Certification
- Must have a valid driving license and a reliable means of transportation to commute.
