Overview
We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team at a reputable Long Term Care Skilled Nursing Facility. The ideal candidate will possess strong clinical skills, experience in various care settings, and a compassionate approach to patient care. This role involves providing comprehensive nursing services to residents, ensuring their safety, comfort, and well-being while adhering to all regulatory standards and facility policies. The LPN will work closely with interdisciplinary teams to deliver high-quality care in a supportive environment.
Responsibilities
- Administer medications and treatments accurately following physician orders and facility protocols
- Monitor and record vital signs, patient assessments, and responses to treatments
- Assist with activities of daily living, including hygiene, mobility, and nutrition support
- Manage IV infusions, catheter care, and other specialized procedures as required
- Conduct physical examinations and assist with diagnostic evaluations using medical terminology knowledge
- Document patient care activities thoroughly using EMR/EHR systems such as Epic, Cerner, or Athenahealth
- Support discharge planning and coordinate with case management for post-acute or home & community care needs
- Provide education and support to patients and their families regarding health conditions and treatment plans
- Ensure infection control practices are strictly followed to maintain a safe environment for residents and staff
- Participate in emergency response procedures including airway management, ventilator support, or acute pain management when necessary
- Maintain compliance with HIPAA regulations and uphold the highest standards of medical documentation
Skills
- Proven experience with long-term care, assisted living, or nursing home environments; ICU, emergency medicine, or trauma center experience is advantageous
- Proficiency in EMR/EHR systems such as Epic, Cerner, or Athenahealth; familiarity with ICD-9/ICD-10 coding is preferred
- Strong knowledge of physiology, anatomy, pharmacology, and medical terminology relevant to geriatrics and complex patient populations
- Ability to perform venipuncture, phlebotomy, injections administration (including immunizations), catheterization, sterile processing techniques, and suturing
- Skilled in patient assessment, vital signs monitoring, medication administration including IV insertion/infusion/tube feeding/ventilator management
- Experience working with diverse populations including pediatrics (neonatal ventilator), behavioral health patients (dementia/dementia care), or individuals with disabilities (developmental disabilities) is desirable
- Excellent communication skills for effective triage, discharge planning, teaching hospital protocols or post-acute care procedures
- Knowledge of infection control standards and aseptic techniques; ability to work within NCQA standards is beneficial
- Ability to work independently as well as part of a multidisciplinary team in a fast-paced environment

Pay: $26.00 - $33.00 per hour
