Your Choice Senior Care West Austin is hiring a Care Coordinator to onboard, train, and evaluate Caregivers, manage care schedules, and support the day-to-day delivery of services. This position is central to ensuring a smooth, responsive, and compassionate experience for every client.
Responsibilities include:
- Support the Administrator in running the overall business
- Onboard new Caregivers with professionalism, warmth, and compliance
- Support clients and Caregivers in the delivery of PAS services, ensuring continuity of care and client and employee satisfaction
- Build extraordinary relationships with clients, families, and referral sources
Duties include:
- Act as Manager when the Administrator is unavailable, following all policies and procedures
Onboard Caregivers:
- Participate in interviewing and hiring Caregivers that meet our needs
- Coordinate new hire (and ongoing) training in accordance with Texas PAS regulations
Communicate with clients and colleagues:
- Maintain the company phone, answer promptly, and use approved scripts & appropriate tone
- Respond quickly, communicate clearly & respectfully with clients, Caregivers, & teammates
- Immediately report all complaints & incidents to Administrator & Alternate Administrator, assisting with issue resolution and follow-up to ensure high-quality care and satisfaction
- Handle after-hours calls, ensuring a designee is identified anytime unavailable
Onboard clients:
- Review Individual Service Plans for accuracy and completeness
- Match clients with appropriate Caregivers and coordinate introductions
Manage schedule of all PAS services (client & Caregiver):
- Maintain a live, accurate schedule in WellSky, ensuring 100% shift coverage
- Provide short-term Caregiver services to maintain continuous operations (as needed)
Ongoing oversight and evaluation:
- Perform quarterly Caregiver Evaluations
- Coach Caregivers by offering positive and critical feedback, preserving the relationship
- Document and upload evaluations, and alert Administrator to ongoing or egregious concerns
- Maintain complete and up-to-date Caregiver records
- Track and log monthly record audits for compliance
Preferred requirements include:
- CNA certification (active or expired) and/or 1 year verifiable caregiving experience w/references
- At least one year of experience in client care coordination, case management, or senior services
- Familiarity with scheduling software and documentation requirements under a PAS license
- Excellent phone presence, empathy, attention to detail, and ability to multitask under pressure

Pay: $41,600.00 - $56,160.00 per year
