Helen Porter Rehabilitation & Nursing is seeking a compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our Memory Care Unit on a part-time day shift schedule. Job Title: Registered Nurse (RN) or Licensed Practical Nurse (LPN)
Location: Helen Porter Rehabilitation & Nursing, Middlebury, VT
Schedule: Part-Time, 8 Hours per Week
Shift: Days, 6:45 AM - 3:15 PM
Weekend Requirement: Rotating Weekends
Unit: Memory Care
As a nurse on our Memory Care Unit, you will:

Provide skilled nursing care to residents living with memory loss and cognitive impairment

Assess, monitor, and document resident conditions

Administer medications and treatments according to physician orders

Support resident safety, comfort, and quality of life

Collaborate with nursing assistants, providers, therapists, and interdisciplinary team members

Communicate effectively with families and care team members

Promote a calm, supportive, resident-centered environment

Advocate for residents while maintaining dignity and respect in every interaction

Current Vermont Registered Nurse (RN) license or eligibility for Vermont licensure

Current Vermont Licensed Practical Nurse (LPN) license or eligibility for Vermont licensure

Experience in memory care, long-term care, skilled nursing, assisted living, rehabilitation, geriatrics, or acute care

Strong communication and interpersonal skills

Ability to work independently and collaboratively as part of a team
