At UConn Health, this advanced working level class is accountable for performing a range of tasks in patient capacity management ensuring safe, efficient, clinically appropriate and timely use of hospital services to optimize patient care and experiences. This position will work in Nursing/Staffing Payroll
SCHEDULE: 64 hours every two weeks 3:00 p.m. to 11:30 p.m., every other weekend and rotating holidays. 30 minute unpaid meal break
SUPERVISION RECEIVED: Works under the general supervision of an employee of a higher grade.
SUPERVISION EXERCISED: Leads employees of a lower grade as assigned.
EXAMPLES OF DUTIES:
Coordinates, exercising advanced clinical judgement, all incoming patient transfer requests from a variety of referring sources including internal and external medical facilities such as hospitals, clinics, urgent care centers, Emergency departments, outpatient providers.
Conducts rapid clinical assessments and determines appropriate level of care for patient placement upon transfer or direct admission.
Facilitates communication between referring providers and UConn Health accepting physicians to secure physician to physician acceptance and to ensure accurate documentation of clinical rationale.
Identifies, assesses and prioritizes time-sensitive or emergency conditions requiring immediate escalation.
Serves as a central communication for patient placement, coordinating with Bed Management staff, nursing supervisors, service line leaders and hospital operations to identify appropriate bed placement for both routine operations and during capacity constraints.
Interprets, applies and ensures compliance with UConn Health policies, procedures, state and federal laws, regulations and standards including Emergency Medical Treatment and Labor Act (EMTALA).
Utilizes electronic and other computerized software, applications, modules and other technologies such as the Transfer Center module to facilitate transfers as well as research, document, analyze, track and report on relevant data; performs ad hoc audits to meet operational and regulatory needs.
Participates in initiatives to improve patient placements in the appropriate care settings at the right time while optimizing hospital capacity; analyze trends and may suggest process and or case coordination improvements.
Performs related duties as required.
MINIMUM QUALIFICATIONS REQUIRED:
KNOWLEDGE, SKILL AND ABILITY:
Considerable knowledge of and ability to apply nursing principles, practices, theory, ethics, and methods.
Considerable knowledge of and ability to apply clinical standards of care, documentation and patient record policies, practices and standards.
Considerable knowledge of and ability to interpret and apply University policies, procedures and best practices as well as federal and state laws, regulations and standards.
Considerable knowledge of and ability to apply clinical assessment and triage principles and practices.
Considerable knowledge of and ability to apply hospital capacity management principles, practices and methods.

EXPERIENCE AND TRAINING:
GENERAL EXPERIENCE:
Graduation from an accredited nursing program.
Three (3) years of professional nursing experience in an inpatient setting such as but not limited to the Emergency department, Intensive Care Unit, Step-Down/Intermediate unit.
SPECIAL REQUIREMENTS:
Incumbents in this class must possess at time of appointment and retain licensure in good standing to practice professional nursing without restrictions in the state of Connecticut.
Incumbents in this class must be able to work in person for long hours, nights, weekends and holidays to meet operational needs.
Incumbents in this class must be able to exercise judgement and critical thinking skills at all times.
PREFERRED QUALIFICATIONS: Critical care or step down level care
WORKING CONDITIONS: Incumbents in this class may be exposed to some disagreeable conditions, stress and demands for critical or urgent actions.
