Elevare Behavioral Health is seeking an experienced and relationship-driven Community Outreach Manager to expand access to behavioral healthcare services and drive sustainable growth across our detoxification, residential, partial hospitalization (PHP), intensive outpatient (IOP), and outpatient programs.
This role is ideal for a business development professional who thrives on building strong referral relationships, creating community partnerships, and helping individuals and families find the treatment they need.
What You'll Do
Drive Admissions Growth
- Develop and maintain referral relationships with hospitals, physicians, therapists, EAPs, case managers, community organizations, legal professionals, and other referral partners.
- Generate qualified referrals that support census growth across all levels of care.
- Execute strategic outreach plans and territory management activities.
- Identify and develop new referral opportunities within assigned markets.
Build Community Partnerships
- Represent Elevare Behavioral Health at networking events, conferences, and community outreach activities.
- Conduct regular in-person visits and educational presentations with referral sources.
- Strengthen relationships through consistent communication and exceptional service.
Support Patient & Referral Partner Experience
- Assist with service recovery efforts involving patients, families, and referral partners.
- Collaborate with Admissions, Clinical, and Operations teams to ensure a positive referral and admission experience.
- Maintain ongoing communication with referral sources throughout the treatment process.
Track Performance
- Maintain accurate CRM documentation of outreach activities and referral sources.
- Monitor referral trends, conversion rates, and market opportunities.
- Provide regular reporting and market intelligence to leadership.
Qualifications
Required
- 3+ years of behavioral health outreach, business development, admissions, or healthcare sales experience
- Strong relationship-building and communication skills
- Proven ability to generate referrals and drive admissions growth
- Valid driver's license and reliable transportation
- Ability to travel throughout assigned territory
Preferred
- Experience in behavioral health, substance use disorder treatment, or mental health services
- Bachelor's degree in Business, Healthcare Administration, Marketing, or related field
- Experience with CRM and EHR systems such as Kipu, Dazos, or similar platforms
- Knowledge of behavioral health levels of care including Detox, Residential, PHP, IOP, and Outpatient

Pay: $75,000.00 - $95,000.00 per year
