As an LPN at Renaissance, you will play an important role in delivering high-quality care to our long-term residents, supporting daily clinical operations, and working closely with RNs and the interdisciplinary care team.

Key Responsibilities:
- Provide direct nursing care to long-term care residents
- Administer medications, treatments, and wound care per physician orders
- Monitor residents’ conditions and report changes to the RN or physician
- Accurately document care and observations in the EHR
- Collaborate with RNs, CNAs, and interdisciplinary care teams
- Support a safe, respectful, and resident-centered care environment
Qualifications:
- Current Illinois LPN license
- Strong clinical skills, organization, and attention to detail
- Experience with medication administration and wound care
- Familiarity with EHR systems (PCC a plus)
- Long-term care or skilled nursing experience preferred
