Legacy Hospice and Palliative Care, located in Chino, CA, is currently looking for compassionate and caring CHHA's to visit our patients in their place of residence in the LAKE ELSINORE and Temecula, Perris, Menifee, Hemet and the surrounding areas
- Per Diem rates range from $35.00 to $40.00 per visit.
- Must be familiar with electronic records system HospiceMD.
- Must have hospice care experience
- Must be willing to cover the areas aforementioned
- CHHA must reside in the area of coverage
Legacy Hospice and Palliative Care, located in Chino, CA, is currently looking for compassionate and caring CHHAs to care for our patients in the Lake Elsinore, Murrieta, Perris or Hemet and areas surrounding***to address patient and family concerns, and work with their team to ensure communication of important information and care plan changes. Must have working knowledge of HospiceMD. Come be a part of our TEAM!!!
Position Qualifications:
- Current licensure to practice in the state of California.
- Current CHHA certification and a minimum of one (2) years experience as a hospice CHHA
- Previous experience in hospice/home care setting
- Proven ability to work within an interdisciplinary setting.
- Complies with accepted professional standards and practice.
- Has excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist, good nursing judgment and communication skills. Understands hospice philosophy and issues of death/dying.
- Understands principles of pain/symptom management.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
- Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills.
- Compiles and maintains current CPR certification.
- Demonstrates excellent observation, verbal and written communication skills.
