Responsibilities:
- The Field Registered Nurse will conduct comprehensive in-home assessments to evaluate each member's health status, care needs, strengths, and personal preferences.
- The Registered Nurse Home Care will develop individualized long-term care service plans that promote independence and quality of life.
- Review medical records, referrals, and supporting documentation to determine eligibility and care needs.
- Accurately document assessments, concerns, and case findings in electronic health records and CRM systems.
- The Field Registered Nurse will coordinate care by identifying and addressing gaps in services to ensure continuity of care.
- Educate members on available healthcare services, community resources, waiver programs, and provider options.
- Collaborate with interdisciplinary care teams to coordinate services and improve member outcomes.
- Communicate with the Call Center and eligible members to facilitate enrollment into managed care health plans.
- Collect quality review data and maintain detailed case notes to support compliance and outcome reporting.
- Identify opportunities for health promotion, disease prevention, and improved member wellness.
- Maintain current knowledge of community resources, payer requirements, managed care guidelines, and available network services.
- Ensure all assessments and documentation are completed accurately and within required timeframes while meeting quality and compliance standards.
Qualifications
- NY RN License
- 2 years of RN experience in a home setting (home health, hospice, UAS nurse, assessment nurse, ect.)
- Must have direct experience conducting comprehensive patient assessments in members’ homes; hospital-only, SNF-only, or clinic-only experience will not qualify
